Health Services

  • The PCSD Health Services Department is dedicated to cultivating a healthy learning environment for all students and staff.  Students, families and the school community are supported through health promotion, education, case management, research, communicable disease management, screenings, referrals, and compliance with the Utah health and welfare laws.

    School Nurse Information for Parents

    Health Concerns: If your student has a health concern that may impact his or her learning, please notify your School Nurse to discuss if a Health Plan is appropriate.  Standard Health Plans including Asthma, Allergy, Diabetes, Seizure, as well as lunchroom accommodations, can be found on the Health Care Plans link.

    Asthma:  Per Utah State Law, a parent requesting a student to carry and/or administer their own asthma inhalers must submit the Asthma Action Plan annually.  Forms need to be signed by both the parent and Health Care Provider.  Inhalers must be labeled with the student’s name.

    Severe Allergies: Per Utah State Law, all students who carry and/or administer their own Epinephrine Autoinjector must submit the Allergy and Anaphylaxis  Plan annually.  Forms need to be signed by both the parent and Health Care Provider.  Epi-pens and antihistamines need to be labeled with the student’s name.

    Medications: Students in grades PreK thru 7th are not permitted to carry over-the-counter or prescription medication on their person.  Students in grades 8-12th may carry a 1 day's supply of non-prescription medication for personal use only, which must be in the original container.  Any medications that needs to be administered by the school must have a district Medication Permission Form signed by a parent and the prescribing provider.
