Transcript and Records Requests
CLASS OF 2023 FINAL TRANSCRIPTS:
Seniors may request an official final transcript be sent to a college or university by entering a final transcript request in their Naviance account. Counselors will be in the 12th grade English classes on May 23 and 24 to show Seniors how to request their final transcripts and complete the "Class of 2023 Farewell" survey in Naviance. Final graduate transcripts will be ready at the end of the day on June 6.***IMPORTANT NOTE about PAPER transcript requests in JUNE: Class of 2023 graduates can request an official transcript be sent to a college via their Naviance account. HOWEVER, if the college does NOT accept electronic transcripts via Naviance (indicated by a “stamp” icon), a paper copy must be mailed. The Registrar’s office can accommodate mailed paper transcript requests until the office closes on June 9. After June 9, graduate records are moved to the District Office and you will need to contact them for mailed paper transcripts (see Graduates section below).
Transcript Requests:
Former students (who did not graduate from PCHS) can request their transcripts or records from the Registrar via email. Graduates can request their transcripts from the District Office or in Naviance. See the "Graduate Transcripts" expandable section below.
Personal/unofficial copies of transcripts or records can be requested directly from the Registrar via email or in the Counseling office (don’t request them in Naviance).
Seniors and Juniors who want an official transcript (signed, stamped, sealed and mailed directly) for colleges or scholarship programs need to enter those requests in Naviance. Refer to the expandable sections below for more information on how to request transcripts for various purposes.
Sophomores who want an official transcript (signed, stamped, sealed and mailed directly) can contact the Registrar via email or in the Counseling office. If the transcript needs to be mailed, be sure to provide the mailing address of the recipient.

Graduate Transcripts
Transcript Requests at the District Office:
- Official graduate records are transferred to the District Office after graduation. Graduates can request their official transcripts and records from Theresa Eason at 2700 Kearns Blvd or theason@pcschools.us or call 435-645-5600 ext 1451 for more information. There is a small fee for each official transcript.
Transcript Requests via Naviance:
- Graduates from the classes of 2016 through 2022 may request official transcripts via their Naviance account. If you have forgotten your Naviance login, contact the District Office to have a paper copy mailed.
- IMPORTANT! Naviance doesn’t send your transcript automatically. A school official sends your transcript after receiving your request in Naviance. Requests made during the summer, after school hours, on weekends, or during holiday breaks won’t be processed until school hours resume. If you need a transcript sent during the summer, or by a certain deadline, please contact the District Office for a mailed paper transcript instead (see above).

Official Transcripts for College Admissions
IMPORTANT! Naviance doesn’t send your transcript automatically. A school official sends your transcript after receiving your request in Naviance. Requests made after school hours, on weekends, during holiday breaks, or during the summer won’t be processed until school hours resume.
Official transcript requests may take up to 5 working days to process. Be aware of deadlines and make sure you’re requesting your transcript well in advance. If you enter a last minute request in Naviance (within 5 days of your deadline), notify the Registrar or your Counselor immediately.
NOTE: Some colleges ask for self-reported grades during the application process and DO NOT require an official transcript. If the college doesn’t want an official transcript with your application, don’t request one be sent. Make sure you are following the college’s application requirements.
Log in to your Naviance account.
Add a College to your “Colleges I’m Applying To” list and Request Transcripts:
- Go to the Colleges I’m Thinking About page (find it on your home page or in the Colleges menu).
- If you are applying to a college already in your list, select the college, then click Move to Application List. If you don’t have any colleges in your Thinking About list, add some, then move them to your Application List.
- Choose the appropriate application deadline for your situation and indicate how you are submitting your application (direct to the institution or via Common App). If you've already submitted your application, check that box. Click the Add and Request Transcripts button.
- On the next screen, request an Initial transcript. (Don’t request midyear or final transcripts until you know you need them--you can do that later. Final transcripts should only be requested for the college you ultimately choose to attend.)
- Click the Request and Finish button.
Request Transcripts for Colleges already in your “Colleges I’m Applying To” list:
- Go to the Colleges I’m Applying To page (find it on your home page or in the Colleges menu).
- Select the College and click on Request Transcripts.
- Request an Initial Transcript if you haven’t requested an initial application transcript yet.
- If your college requires a Midyear transcript (in addition to the initial one we already sent), or a midyear progress report, or you are applying via Common App, contact your Counselor directly to make a Midyear transcript request.
- DO NOT request Final transcripts until you have been accepted by the college and have made the decision to attend. Counselors will be in the 12th grade English classes in May to show Seniors how to request their final transcripts.
- Make sure the correct college is listed and click Request and Finish.
Use the Manage Transcripts page to see if/when your request was processed. If you haven’t seen any movement on your request, contact the Registrar or your Counselor to find out why. DON’T request duplicate transcripts in Naviance! Common App requests require your Counselor to gather additional information before the transcript can be sent, which takes more time. If a college in your application list has an unknown delivery type icon (a question mark computer screen), you first need to indicate how you will be applying to that college (via Common App or direct to the institution) before we can send your transcript.
It can take up to 10 days for colleges to process transcripts and update their application systems. If you get an email or their application portal says they haven’t received your transcript, but Naviance says it was mailed, contact the college directly via phone, email, or chat and ask them to double check. If they still haven’t received it, contact our Registrar to have another one sent (don’t add another request in Naviance).

Official Transcripts for Scholarships, Athletics, HSUP, etc.
IMPORTANT! Naviance doesn’t send your transcript automatically. A school official sends your transcript after receiving your request in Naviance. Requests made after school hours, on weekends, during holiday breaks, or during the summer won’t be processed until school hours resume.
Official transcript requests may take up to 5 working days to process. Be aware of deadlines and make sure you’re requesting your transcript well in advance. If you enter a last minute request in Naviance (within 5 days of your deadline), notify the Registrar immediately.
NCAA Transcripts: Register with the NCAA Eligibility Center and a transcript request will automatically go to our Registrar. You DON’T need to put a request in Naviance. For more information, see the NCAA Transcripts expandable section below.
Scholarship and Other Transcript Requests:
- Log in to your Naviance account.
- Click on the Colleges menu and select Manage Transcripts.
- Click the round red + icon, then choose Other Transcript.
- Complete the entire form and add any special notes. You must provide ALL the correct address information or we won’t know where to send it! (Don't request transcripts for yourself in Naviance--contact the Registrar directly for personal copies.)
- Click Request and Finish at the bottom.
Use the Manage Transcripts page to see if/when your request was processed. If you haven’t seen any movement on your request, DON’T put in another request. Contact the Registrar to find out why. UVU and WSU Concurrent Enrollment
If you took classes at PCHS for concurrent enrollment college credit, and you want to have those UVU or WSU college credits sent to a college or scholarship program, you need to contact the Registrar's office at Utah Valley University (801-863-8468) or Weber State University (801-626-6100) directly. We do not have access to your college transcripts.
If you are signing up for concurrent enrollment courses and need your transcript and ACT scores sent to UVU or WSU for course prerequisites, go to the Concurrent Enrollment page and locate the Transcript Request Form link.

ACT, AP, and SAT Test Scores
Colleges will most likely require official ACT, AP, and SAT test scores, which must be ordered and sent directly from the testing agency. Refer to the testing agency's website for information and contact them with questions. Be aware of deadlines and how long it can take for the testing agency to send your scores.

PCHS might have your unofficial test scores. If you want a printout or photocopy of any scores the school may have in your file, contact the Registrar.

NCAA Transcripts
If you plan on playing Division I or II athletics, be sure to register with the NCAA Eligibility Center. That will automatically create a transcript request that will go to our Registrar. If you’re registered with the Eligibility Center, a final end-of-year transcript will automatically be sent at the end of both Junior and Senior years.
You DON'T need to put NCAA transcript requests in Naviance. Transcripts will be processed and sent electronically through the NCAA Eligibility Center.
IMPORTANT: If you are taking Utah Students Connect courses, DO NOT list Utah Students Connect as a separate school on your NCAA account. You should only list Park City High School--we are the school that awards you the credit for UTSC courses.
